Assisted Living Tasks : (Only 19 residents in this location – 13 in traditional AL and 6 in AL Memory Care)

Functional Capacity Screen
Risk meeting
RD Recommendations
Assessments (can be delegated to a nurse when we can): AIMS, Bed Rail Assessment, SW Braden, Cornell Scale for Depression in Dementia (CCSD) Memory Care only, Morse Fall Senior Living, Pain Tool Senior Living, Elopement screen.
This role would also participate in the on-call rotation shared with all managers. On-site housing is available in our Independent Living community.
Census/Bed Size: 60 beds
Shift/Schedule: M-F
