Planet Healthcare/GHR is seeking a Home Health Physical Therapist for a 13-week contract (with the option to extend) for our client partner in Escanaba, MI.

Requirements:

• Minimum 1+ year experience as PT
• Minimum 6+ months in a Home Health setting as a PT
• OASIS and Home Care Home Base experience
• Current licensure in state of practice
• Current CPR certification required
• Current Drivers' License, vehicle insurance, and access to a dependable vehicle or public transportation

Duties may include:

• Evaluate patients in their homes and develop individualized treatment plans based on physician referrals, patient goals, and home environment considerations.
• Educate patients and caregivers on exercises, mobility strategies, and use of assistive devices (e.g., walkers, canes).
• Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
• Document visits and treatment outcomes accurately for compliance with medical, insurance, and legal standards.
• Collaborate with interdisciplinary care teams, including nurses, occupational therapists, and physicians.
